The Community Case Manager position is located within our Ruah Intensive Team focusing on working with people with a combination of serious health and social disadvantaged circumstances.
The Ruah Intensive Program is looking for Community Case Workers to work with people with chaotic and complex needs. Individuals worked with can include complex presentations of mental illness, substance use, HIV, HCV, social marginalisation, homelessness and/or transient circumstances. The Intensive Program provides psychosocial and rehabilitation support in an assertive community case management role. The service works in conjunction with a range of medical services. These positions are located in Leederville.
Community Case Manager Positions: a relevant tertiary degree is essential. Salary: $40,330 to $50,300 depending on experience.
