Ruah Assertive Outreach Team
The Street to Home; Ruah Assertive Outreach Team is located at the Ruah Centre, an agency of Ruah Community Services. The Street to Home Program is a multi-agency collaboration between 8 different specialist homelessness services and a mental health clinical outreach team. All agencies work in partnership to enable people experiencing primary homelessness to move off the street to stable housing and an improved quality of life. Initial engagement of all program clients occurs through the Assertive Outreach component of S2H Program.
Who can use the Street to Home Program?
• People 18 years of age+ who:
o Are primary homeless (rough sleepers) without conventional accommodation
(living on the streets, in deserted buildings, improvised dwellings, in parks,
laneways and so on.)
o Qualifies for a Centrelink benefit
o In Perth or Fremantle inner city areas (Fremantle area covered by St Patricks)
Hours and Mode of Work
Street to Home Assertive Outreach Workers will have the capability to undertake street based shifts to connect with the client group, at different times during the days dependent on the needs of the client group, this may involve early morning- day- evening shifts depending on each agency.
** Please note the Street to Home Program is not a crisis response service.
Ruah Centre Assertive Outreach Workers will:
• Develop relationship to gain engagement
• Tailored & flexible support plans
• Connect to existing day centre services
• Collaborate with mainstream and other agencies
• Partner with mental health clinical outreach team
• Link to supported housing (SH) providers
• Provide transitional support until transfer of case management with SH providers
